Chain of Custody

All samples submitted for analysis must have a completed chain of custody form. If you do not have a chain of custody form, please download a copy of ours for your sample submission. We can also custom design and produce a chain of custody form for your company. Give us a call at (801) 964-2511. There is no charge for this service for Enviropro Laboratories clients.
